New Client Information

You may be new to the area, new to pet ownership, or unhappy with your previous veterinary care. In any case, Olde Towne Animal Hospital is excited to welcome you to our practice. We know that you’ll appreciate our standard of care, helpful veterinary team, and unmatched medical facility. We can treat your pet with a wide array of services, including medical, diagnostic, surgical, and dental care.

Before you meet with our team, we ask that you fill out a few forms so we can get to know you and your pet. We have a new patient form for all clients and a boarding agreement form for cat owners who want to board with us. You can print these forms off, fill them out, and bring them to your first appointment. Or, submit them electronically via our website. This background information will allow us to prepare and ensure the appointment runs quickly and smoothly.

At your first appointment, we’ll take the time to get familiar with you and your pet. First, our team will go over your new patient form and ask questions about your animal’s medical history. From there, we can evaluate his physical condition. We will be sure to check his eyes, ears, heart, lungs, and teeth as a touchstone for his health moving forward. If we find any areas of concern, we can perform additional tests onsite.

We hope to build a relationship that will last for years to come. Together, we will create a health plan that works for you and keeps your pet in optimal health.
